Health Declaration

Please fill out the following form to help us understand your physical condition and assess your fitness to train.

Before taking part in any physical activity, please read the questions below and select any that are relevant to you or check 'none of the above'. If you are over 69 years of age and you are not used to being very active, please consult your GP before starting a new exercise program

If you answered "No" to ALL of the questions: It is reasonably safe for you to participate in physical activity.

 

If you answered "Yes" to one or more of the questions: You should consult with your doctor to clarify that it is safe for you to become physically active at this current time and in your current state of health BEFORE you start a new exercise program.  You may be able to participate in any activity you want – as long as you start slowly and build up gradually. Or, you may need to restrict your activities to those which are safe for you. Talk with you doctor about the kinds of activities you wish to participate in and follow his/her advice.

Delay becoming more active:

  • If you are not feeling well because of a temporary illness such as cold or a fever – wait until you feel better.

  • Please note: If your health changes so that you then answer YES to any of the above questions, tell your fitness or health professional immediately.

LIABILITY WAIVER
I am registering to participate in activities held by Bfit.

The instructions and advice presented are in no way intended to be substitute for medical counselling/advice. Not all exercises are suitable for everyone and this or any other exercise program may result in injury.

Any exercise program carries a risk of injury.  Every step has been taken to ensure you train safely, however, if at any point during your workout you begin to feel faint, dizzy or have physical discomfort, you should stop immediately and consult a medical professional. Any user of Bfit assumes the risk of injury resulting from performing the exercises shown. Bfit hereby disclaim any liability or loss in connection with the exercises and advice contained herein.

I, the undersigned, hereby state that I wish to participate in the exercise class / course / workshop. I realise that my participation in these activities involves potential risk of injury. I hereby state that I am in good health and am able to take part in this activity. I take full responsibility for my own health and well being and agree that any instructions received will be used at my own risk.

I have disclosed above any relevant medical information and will inform the instructor if there are any changes to my health as stated above before partaking in any further training.

My signature also acknowledges my consent to be photographed and/or video recorded during the class, and grants the instructor the sole right to own, use and modify the images and/or recordings for any and all purposes worldwide, including but not limited to advertisements, marketing, and all commercial purposes and in all forms of media, without any compensation (including royalties) in perpetuity. I waive any right to inspect or approve any images or recordings and waive any rights of ownership to the same.
